Cariprazine in Bipolar I: Depressive Episodes and Dose Escalation

A summary of the licensed dosing in bipolar depression, and why escalation is deliberately slow.

Licensed dosing

For depressive episodes associated with bipolar I disorder, treatment starts at 1.5 mg once daily. It may be increased to 3 mg on day 15 if warranted. The trials showed benefit at both doses.

Why escalation is slow

Cariprazine and its active metabolites have a long half-life; steady state takes one to two weeks to reach. Escalating faster does not reach effect sooner — it front-loads akathisia, which is the most common reason people stop.

Akathisia

Inner restlessness, an inability to sit still. It is dose-related and usually emerges in the first two weeks. It is manageable — dose reduction, sometimes a beta blocker — but only if it is recognised, and it is frequently mistaken for the agitation of the underlying illness.
